BATON ROUGE, La. – Blue Cross and Blue Shield of Louisiana recognized clinics and primary care providers statewide for their performance in the insurer’s Quality Blue value-based care program.
“In Quality Blue, we work closely with provider partners around the state to get better outcomes for our shared customers – their patients, our members,” said David Hochheiser, Blue Cross senior vice president of Provider Networks and Healthcare Value. “In the seven years since we began Quality Blue, our shared focus on care coordination, managing chronic conditions and financially rewarding providers for taking extra steps to keep their patients healthy has led to improved health and lives.”
In Quality Blue, Blue Cross establishes clinical quality measures for four chronic conditions that are common in Louisiana – diabetes, heart disease, high blood pressure and kidney disease – that track patients’ health improvements over time. For example, providers are tracked to see how many of their patients have blood pressure or blood sugar readings in a healthy range, are taking medication as directed, or are not smoking.
Blue Cross supports primary care practices that participate in Quality Blue with training, tools and support – including hands-on help from Blue Cross’ own staff. Blue Cross shares health data securely with Quality Blue primary care providers. This helps the providers identify which of their patients are most in need of interventions like preventive screenings, tests or referrals for health coaching with Blue Cross’ in-house team of clinicians. It also lets Blue Cross and providers work together to close gaps in care and prevent hospital admissions, emergency department visits and other intense, high-cost treatment needs.
At its annual Quality Blue Statewide Collaborative on Nov. 10, Blue Cross recognized clinics with the highest achievement in the program for 2020. Normally, Blue Cross would host an in-person event, but because of the COVID-19 pandemic, this year’s collaborative was virtual.
Blue Cross recognized three clinics with the highest scores on Quality Blue’s clinical quality measures for treating patients with three of the program’s targeted chronic conditions:
Because of changes to the Quality Blue program measures that took effect earlier this year, there was no Highest Achievement award presented for Diabetes Care.
Blue Cross also recognized two clinics that made the greatest overall improvements on the quality care measures with a 2020 Momentum Award:
